Only online access is free ($0.00), patient is responsible for any checkup and treatment done. Will be confirmed either by text message, or phone call or email.
2. Cancellation & Rescheduling
Patients must cancel or reschedule appointments at least [24/48] hours in advance to avoid cancellation fees.
A late cancellation or no-show may incur a fee, as outlined in our cancellation policy.
3. Payment & Insurance
Some treatments may require a deposit at the time of booking.
Insurance claims are processed separately, and coverage must be verified before treatment.
Any outstanding balances must be settled before scheduling new appointments.
4. Patient Responsibilities
Ensure that the contact information provided is accurate.
Arrive on time for scheduled appointments.
Inform us of any medical conditions or changes in health history before treatment.

All personal and health information submitted through the booking system is protected in compliance with HIPAA regulations.
We do not share patient data with third parties without explicit consent.
6. Office Rights
We reserve the right to modify appointment availability or cancel appointments due to unforeseen circumstances.
Patients who repeatedly miss appointments may be restricted from online booking privileges.
