3. Payment & Insurance

• Any outstanding balances must be settled before scheduling new appointments.

4. Patient Responsibilities

• Ensure that the contact information provided is accurate.

• Arrive on time for scheduled appointments.

6. Office Rights

• We reserve the right to modify appointment availability or cancel appointments due to unforeseen circumstances.
